As of January 1, 2012, you no longer contact the County you reside in for assistance.  You are now required to contact the 'District' you reside in.  Please take a moment to look at the Wisconsin Consortia Map to determine your district and call the number listed.

You will be provided enrollment services and can manage your benefits for:

• FoodShare
• Medicaid
• BadgerCare Plus
• Family Planning Only Services
• Caretaker Supplement
