Identification Required
Identification is essential to receive food.
We require that the individual picking up the food has to prove both their name and address at every visit.
PROOF OF NAME: This can be anything with a picture I.D., such as a driver’s license or Wisconsin identification card.
PROOF OF ADDRESS: This should be something “other” than a driver’s license. Example: pay stub, current utility bill, current rent receipt, letter postmarked within the past 30 days or letter of referral from a Government or Church agency (Referrals must be in writing. We do not accept referrals via the telephone).
On your FIRST initial visit, you'll be required to provide proof for EVERY person residing at your address. For household members other than the applicant, documentation is needed only once.
If the applicant does not provide documentation for each household member by the third visit, then the pantry will only continue to provide food for every member for whom documentation has been provided.
Acceptable documents include:

- Wisconsin Immunization Registry records
- Report cards
- Birth or baptismal certificates
- Passports
- Medical, insurance, or library cards
- Driver’s licenses
- State/military/tribal IDs
- First-class mail to a member of your household and postmarked within the past 30 days.
- First-class mail postmarked in the past 30 days
- Utility bill
- Rent receipt, lease agreement or a property tax bill
- Room key – if living in motel/hotel
